About the data

Every rating, measure, address and phone number on this site comes from the Centers for Medicare & Medicaid Services (CMS) Provider Data Catalog (data.cms.gov), a U.S. government source in the public domain. Latest releases we use — dialysis facilities: 2026-06-16, home health agencies: 2026-05-27, hospices: 2026-08-19. We check for new releases every week.

Dialysis star ratings (1–5) are calculated by CMS from outcomes such as patient survival, hospital admissions and readmissions, transfusions, bloodstream infections and dialysis adequacy. Outcome labels ("better than expected" and so on) compare a center with what CMS expects for its patients. Home health quality star ratings summarize how often patients improved and how timely care began; patient survey ratings come from the Home Health Care CAHPS survey. Hospice ratings come from the CAHPS Hospice Survey of family caregivers and from the Hospice Care Index (claims-based, 0–10).

Service areas. Home health agencies and hospices report the ZIP codes they serve; a city page lists every agency that lists at least one of the city's ZIP codes. Dialysis city pages list centers within 10 miles of the city center, measured from the center's ZIP code.

Doctors and other clinicians come from the CMS Doctors and Clinicians National Downloadable File: everyone enrolled in Medicare with a practice address, their specialty, telehealth and whether they accept Medicare assignment. The MIPS final score (0–100) comes from the Clinician Public Reporting file for the latest performance year; hospitals come from the Facility Affiliation Data file. A city page lists clinicians whose practice address is in that city. Payments from drug and device companies come from CMS Open Payments (general payments of the latest published year, summed by NPI).

We do not rate providers, accept payment to change any rating, or give medical advice. Sponsored listings are labeled. We are not affiliated with CMS or Medicare.
